Long
Term Care Facility Advisory Board To Hold Public Forums
Two of the four meetings of the Long Term Care Facility
Advisory Board, beginning in Fiscal Year 2005, will offer a public
forum for interested parties to provide the board with trends
and issues that would be of concern to the board. The public is
encouraged to present their views on how the system is working
or not working for families and residents of long term care facilities.
The first public forum is scheduled for July 28,
2004 at 10:00 a.m., at the Crown Plaza Silversmith, 10 South Wabash
Avenue, Chicago, 60603. (312.372.7696)
The Board meetings that will have the public forums
will be organized as follows:
| 10:00 a.m. to 12:00 p.m. |

| 1:00 p.m. to 3:00 p.m. |
Board Meeting |
Those interested in making presentations can contact
the Department of Public Health in advance and inform the Department
of the topic they wish to discuss. Please contact Susan Quinn
at 217-785-9181 to schedule a presentation.
Those who do not contact the Department in advance
may fill out a form prior to the meeting asking to make a presentation.
The form must be filled out, or the public speaker must be on
the agenda in order to be acknowledged and make a presentation
to the Board. The amount of time allotted per presenter will depend
on the number of persons presenting on that day.
If questions are asked, they will be answered to
the extent possible. If questions cannot be answered at that time,
the Department will provide a response to the questions by the
next board meeting.
Any member of the public may attend any of the board
meetings.
